Youth 2 Youth Geauga Dodge ball Tournament Registration!

If you wish on participating/watching and you are a High School student, this form needs to be completed by October 12th.
This fundraiser is to financially aid the Red Tulip House and to help out Geauga Youth 2 Youth so we are able to have more events like this.

If you would like to come, it is $5.00 to play and $2.00 to watch. Participants MUST arrive no later than 1:30 p.m.!

The tournament will be held at the Geauga YMCA (12460 Bass Lake Rd, Chardon, OH 44024) on October 26th from 2-4 pm. Bring cash because there will be concessions! Can’t wait to see you there!

ALL PARTICIPANTS UNDER THE AGE OF 18 MUST HAVE A PARENT/GUARDIAN TO SIGN A RELEASE FORM IN ORDER TO PARTICIPATE! PRINT THE PERMISSION SLIP BELOW AND BRING IT WITH YOU THE DAY OF THE TOURNAMENT! IF YOU DO NOT HAVE THIS YOU CAN NOT PARTICIPATE IN THE TOURNAMENT. THIS CAN RESULT IN YOUR TEAM GETTING DISQUALIFIED!
