Full-Time Residential Case Manager

The Residential Case Manager provides supportive activities to enhance a resident’s long-term recovery from substance use disorders. They must be knowledgeable about Substance Use Disorders and community resources, and preferably have experience in residential treatment settings. The Residential Case Manager assists residents in gaining access to needed health and dental services, financial assistance, housing, employment, education, social services, and other services and supports developed through the individualized treatment planning process. They interact with residents in a supportive manner, encouraging discussion and activities focused on recovery.  Responsible for scheduling and transport to medical, mental health appointments and other transportation as necessary; assists with screening and intakes of potential residents, assessments, treatment planning, case management, individual and group counseling as assigned (depending on licensure).  Acts as liaison with referral sources, planning, linkage and coordination of referrals to various support services; monitor residents progress in services; provide client advocacy.  Must be able to manage normal residential treatment functioning and emergency situations independently while maintaining professional standards of ethics. Completes client charting and other required documentation and accurately tracks outcomes and completes residential treatment follow-up reports as part of agency Quality Improvement Plan.

High School Diploma/G.E.D. required, Undergraduate degree in human service area preferred, CDCA licensure and Valid Ohio driver license required.

Background Check and Alcohol and Drug Screen required.

Pay: $28,000.00 – $42,000.00 per year
