Community Health Survey
The Lake County Community Health Advisory Board has recently implemented the community health assessment survey.
Interested individuals may complete the survey at www.lakecountyhealthsurvey.org. Individuals completing the survey will be able to enter a raffle to win one of several prizes ranging from a Nintendo Wii, Wii Fit, and a variety of gift certificates. LCGHD and its partners will also be at several locations in Lake County giving the survey to the public in order to get the highest level of participation as possible.
